Licensed Clinical Therapist
Duties & Responsibilities
§ Complete assessment and intake services in a timely fashion for all assigned clients.
§ Develop individualized service plans in conjunction with the family and other agencies/professionals.
§ Document all services in accordance with Community Works policy and procedures and submit in a timely fashion. Ensure Medicaid and insurance guidelines compliance, as applicable.
§ Plan and administer therapeutic treatment identified in the individualized service plan. Change method and degree of therapy when indicated by client reaction/needs.
§ Collaborate with other professionals, family, and agency clinical staff to ensure consistency and timely completion of goals identified in the ISP.
§ Maintain regular contact with other agencies/professionals and family regarding client progress via phone calls, letters, emails and/or monthly reporting. Prepare court reports as needed.
§ Attend agency required meetings; i.e. staff meetings, treatment team meetings, IEP meetings.
§ Comply with certification/licensing standards and provide all related documentation to administrative office.
Comply with all policies and procedures of the agency, support the mission and vision of the agency, follow all guidelines of Employee Code of Conduct.
§ Master’s degree in human services field;
§ Current license with the State of Oklahoma
§ At least 2 years experience in human services field
§ Must be able to lift & carry up to 25 pounds
§ Full range of body motions
§ Ability to spend 80%-90% of time standing, walking, kneeling and/or reaching to perform office duties;
§ Sight, hearing, touch, & speaking is required 100% of time to perform office duties & interact with clients, consumers
§ Approved OSBI Criminal History Record
